We see patients by appointment. If you have an urgent issue, we will do our best to accommodate you the same day. If you are unable to keep your scheduled appointment, we ask that you give us 24 hours notice so that we may allow someone else the opportunity to reserve that time.
Please bring your insurance card, valid photo id and a list of any medications you are currently taking to your appointment.
We recommend arriving 15 minutes early for your appointment.
When calling for a prescription refill, please provide us with the name of the medication you need refilled, the name of your pharmacy and the pharmacy phone number.
Please allow 1 business day to refill prescriptions. We do not refill prescriptions after hours or on weekends.
Some prescription refills may warrant another visit to our office before they can be refilled. If this is necessary, someone from our office will contact you to let you know this.
Financial / Payment
We accept most major health insurance plans including those listed in the section below. If you do not see your insurance listed below, please contact us because it is possible that we may be in the process of enrolling or may consider enrolling.
All co-payments and deductibles are the responsibility of the patient. For your convenience, we accept cash, debit and credit cards. Checks are not currently accepted.
